Purchase Order Information for Non-DFCI Users
Non-Dana-Farber users, or users who pay with a purchase order, will be charged outside rates. (See rate information here.)
We prefer you have a purchase order number when you make your appointments. If you can not have one in time, you can temporarily type "Will Bring" into the "Project Number" box. You will have to manually enter in a number for your appointments afterwards.
For all PO numbers, we require a hard copy with the following information:
- Name, PI, phone, and email of authorized requistioner(s)
- Name and phone number of administrative contact
- Billing address (preferably to your institution's accounts payable)
- PO number
- Amount of money on PO
- For standing POs: expiration date
